A Disease Management Program (DMP) is a program that ensures the treatment and education of certain diseases.

Our practice offers DMPs for the following conditions:

• Diabetes mellitus
• Coronary heart disease (CHD)
• Chronic obstructive pulmonary disease (COPD)
• Asthma

There are no costs for our patients to participate in these programs.
