Electronic Patient Record
We need to transfer data from your current treatment into your electronic patient record (ePA) if you have one. This includes test results and therapy reports we've conducted on you. Laboratory findings and letters to other doctors are included.
We will enter further information upon request, as long as it is available and was collected by our practice.
You have the right of objection. That's important for sensitive information. For example, data related to mental illnesses, sexually transmitted infections or abortions.
